The following form can be downloaded to your computer and printed at home. By filling out this form ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.

Health Insurance Plans

Our office participates with most insurance plans. We will submit all insurance forms electronically and we also pre-authorize all necessary treatments for our patients.

At the time of visit, patients are responsible for deductibles, co-payments and remaining balances.  

Financial Policy

We offer paid-in-full discounts and extended payment plans.  Please call for additional information.

Co-payments and co-insurances are due at the time services are rendered.  Forms of payment accepted by the office are check, cash, insurance assignment and the following credit cards:
